The Rank One Sport Online Forms for 2020-2021 are available.

After reading the information below, please click the link above to access the required student-athlete participation forms.  

Please read the instructions on the home page carefully. Download and print the UIL official medical history/physical exam form to take with you to your health care provider.  Middle school students give your completed physical form to your sport coach. A new physical will be required for the 2020-2021 school year. 

The Electronic Participation Forms to complete for 2020-2021 school year are the Medical Emergency Card and the Insurance/UIL Signature Page.

After completing these steps, student-athletes are now ready to participate in athletics for the 2020-2021 school year.
